Non Illinois Resident Fingerprinting Procedure

• Obtain a fingerprint card from the state agency that is requiring you to be fingerprinted.
• Get fingerprinted using ink by a local law enforcement agency.
• The card must be filled out completely.
• Reason Fingerprinted should specify which license is being applied for (nursing, PERC, etc.)
• Send completed Card and $60.00 money order, made out to Accurate Biometrics. Please include the full name of applicant on the money order.
• You must include your name and phone number in case we need to contact you regarding your fingerprint card
• Send Card, payment and contact information to:

  Chicago, Illinois, 60630
• We will process your card and send it electronically to the Illinois State Police for the background check.  The results will be forwarded on to the required agency. 
• Do Not send any part of your application to Accurate Biometrics. Applications must be sent to the state agency that issues the license.
